Non-NHS work and fees
The Government’s contract with GPs covers medical services to NHS patients, including the provision of ongoing medical treatment. In recent years however, more and more organisations have been involving doctors in a whole range of non-medical work.
Sometimes the only reason that GPs are asked is because they are in a position of trust in the community, or because an insurance company or employer wants to ensure that information provided to them is true and accurate.
Examples of non-NHS services for which GPs can charge their own NHS patients are:
- accident/sickness certificates for insurance purposes.
- reports for health clubs to certify that patients are fit to exercise.
- pre-employment medicals, as requested by employers.
Examples of non-NHS services for which GPs can charge other institutions are:
- life assurance and income protection reports for insurance companies.
- reports for the Department for Work and Pensions (DWP) in connection with disability living allowance and attendance allowance.
- medical reports for local authorities in connection with adoption and fostering.
GPs may not be obligated to perform non-NHS work for patients, except in certain limited cases like confirming a patient’s inability to serve on jury duty. Although GPs may try to help patients with tasks like completing insurance forms, they are not mandated to do so. Patients may be charged for these services, and appointments for such purposes need to be scheduled separately.
It’s advised not to book routine appointments for non-NHS work to avoid potential inconvenience.

An employer or another agency might suggest that patients “get a letter from your GP”, but this is not always possible or appropriate. If it is, it will likely incur a cost, as the NHS does not pay doctors to undertake this work.
The following lists explain more about the nature of our non-NHS service provision (noting that the list is not exhaustive):
Help We Can Provide
- Fit Notes
Officially called MED3 certificates, but most commonly known as ‘sick notes’. These are ONLY issues for an illness lasting 7 days or more, and ONLY for the purposes of obtaining sick payment benefits.
For sickness lasting less than 7 days, we will not provide a sick note. If your employer insists then there will be a charge for this.
- Other Statutory Certificates Required By Law Or A Referral Letter
Referral letters to hospitals, secondary care and wider healthcare providers, both NHS and private, and some benefits application forms.
- Statement Of Fact Or Provision Of Medical Services
Sometimes, patients are required to provide evidence of a medical or clinical fact / condition (e.g. confirmation of vaccinations).
- Travel Cancellation And Insurance Claim Forms
We are happy to complete travel cancellations forms if we have been caring for/treating you for a condition for which we have advised thar you cancel or postpone upcoming travel. We will also complete insurance claim forms.
- Fitness To Travel
Fitness to travel letters from a GP are generally not required. However, for uncomplicated cases only, we can provide a short letter, which is essentially a brief statement of fact and a comments that we can see no contraindications to travel at the time of writing. We are not in a position to provide medical opinion, or to complete a complex pro forma regarding travel.
- HGV, PSV And Taxi Driver Medical Examinations
These forms require a long, face-to-face appointment. Please hand in the form to reception and a member of our team will contact you regarding an appointment and charges.
- Other Medical Examinations
Some medicals, e.g. for employment purposes, fitness to travel, insurance claims etc, require a more detailed face-to-face examination. Our time and fees reflect this. . Please hand in the form to reception and a member of our team will contact you regarding an appointment and charges.
- Medical Records
You have the statutory right to review your medical records. An application must be made in writing to the practice. You may also have copies of your records. There is also a lot of information on your NHS app. We will provide copies of your medical record only if you cannot view your records via the NHS app.
- Medical Reports
We will provide medical reports on written requests, from employers. These reports are provided on the basis of our role as the patient’s family GP only, and not from a professional occupational perspective. Should such a report be insufficient for your employer’s needs, we recommend that they seek the advice of a qualified occupational health physician.
- Sports Medicals
We can provide a short letter, which is essentially a brief statement of fact and a comment that we can see no contraindications to undertake sport at the time of writing. We are not in a position to provide medical opinion, or to complete a complex pro forma regarding travel.

- Passport Or Driving License Endorsements
- Character References For Interview Or Job
- Fit For Exercise Of Gym
We encourage all our patients to exercise as it is generally very good for you and an excellent treatment for many common conditions. The risk that harm would come to you by exercising is low, however we cannot say that it is 100% safe for anyone / everyone to exercise. As such, the risk is entirely between you and your gym instructor, following the guidelines of a specific gym.
- Missing A Court Appearance Or Probation Letters
We do not provide these unless we have been personally directed to do so but a Court Order. Your solicitor will advise further.
- Missed Or Deferred Exams, Schooling Or University Letters
There is no obligation on GP’s to provide sick notes or letters for educational institutions (e.g. for menstrual periods, toilet breaks, term work or dietary requirements). It is the responbility of the parent / guardian or student to provide this information.
For missed exams, a school might be able to reques special consideration. Please see the Joint Council for Qualification (Home – JCQ Joint Council for Qualifications – access arrangements and special consideration section).
- Benefits Appeal Letters
GP’s are not responsible for making decisions on peoples’ benefits entitlements; accordingly the Department for Work and Pensions (DWP) already has process in place to request the appropriate information from GP’s to enable them to make decisions.
- Recording Injuries For Police Or Insurance Purposes
In the case of an alleged assault, a police surgeon must assess and document injuries. This is not an NHS service and we are not trained to do this work to the standard required for the courts.
Likewise, attending the surgery for documentation of injuries obtained from a road traffic accident is not an NHS service, and it is not appropriate.
- Approval Of Dieting
We do not recommend any diets that are non-physiological e.g. extremely low-calorie diets or diets that consist of only one food type. A healthy, balanced diet in conjunction with regular exercise is the only diet we recommend, and this does not require a letter of permission from your doctor.
- Fitness For Operations Or Dental Work
It is the responsibility of the professional or organisation carrying out the work to determine for themselves whether you are fit for a particular procedure or operation, minor or otherwise. We cannot provide any guidance or authorisation for the professional or organisation concerned. If you believe it would assist you, we can provide you with a pint out of your medical record.
- Fitness For Diving
Diving medicals are a highly specialised area of medicine, and your diving instructor can advise you on how to find a suitably qualified doctor to assess your fitness to dive. If you believe it would assist you, we can provide you with a pint out of your medical record.
- Other Private Work
The nature of private work is that we are not bound by any obligations to provide it. Therefore, we reserve the right to decline any private work which is presented to us.
